PROSALUD HMO CORP

The Court of First Instance, Superior Chamber of San Juan, in Civil Case No. KAC2009-1424 has initiated liquidation proceedings against the health services organization PROSALUD HMO, Corp. For these purposes, by Order issued on December 21, 2009, the Court appointed the Puerto Rico Insurance Commissioner as the Liquidator of PROSALUD.

By virtue of this Order, any contract issued by PROSALUD, which was in force on the date of the Liquidation Order, will be canceled. However, subscribers of this plan are informed that the Office of the Insurance Commissioner authorized the transfer of PROSALUD subscribers to other health care plans in order not to interrupt the health services that were formerly offered by this organization.

PROSALUD subscribers, providers, claimants and/or creditors are also informed that they must submit their claim using a Claim Form, which will be available at the Office of the Insurance Commissioner or at the offices of PROSALUD.

Any unpaid claim must be submitted again using the Claim Forms, regardless of whether it was submitted before the date of the Settlement Order.

The completed Claim Form should be sent to the Liquidation Office, Avenida Eleonor Roosevelt, 122 Interior, San Juan, P.R. 00918-3305, no later than February 28, 2010. If proper evidence is not claimed or presented within the period set out here, it will be understood that the right to complain has been waived.
