The Court of First Instance, Superior Chamber of San Juan, in Civil Case No. KAC09-1569 (905) has initiated liquidation proceedings against the health services organization SDM Healthcare Management, Inc. (WITHOUT). For these purposes, by Order issued on December 17, 2009, the Court appointed the Insurance Commissioner of Puerto Rico as the Liquidator of SDM.

By virtue of this Order, any contract issued by SDM, which was in effect on the date of the Liquidation Order, will be canceled.

SDM providers, claimants and/or creditors are also informed that they must submit their claim using a Claim Form, which will be available at the Office of the Insurance Commissioner or on SDM premises.

Any unpaid claim must be filed using the Claim Forms, regardless of whether it was submitted before the date of the Settlement Order.

The completed Claim Form must be submitted to the Settlement Office, PO Box 363031, San Juan, P.R. 00936-3031, no later than February 28, 2010. If proper evidence is not claimed or presented within the period set out here, it will be understood that the right to complain has been waived.
